Coaches: This Official Visit Request Form must be completed in its entirety and approved by the Compliance Office in order to allow for travel and lodging arrangements to be made for a prospective student-athlete for an official (paid) visit on the University of Jamestown campus. Coaches are responsible for arranging travel and lodging. This form must be submitted to the Assistant AD for Compliance at least 72 hours before the prospective student-athlete's arrival on campus. Additionally, in regards to the prospective student-athlete's transcript, you will need to ensure that there is a current transcript on the Eligibility Center, or a current copy of the transcript has already been evaluated by Compliance and returned to you, or you must send an email of a current transcript to Compliance before the visit will be approved.
